Merit Badge Counselors - Annual renewal form

This form is for existing merit badge counselors to confirm or update their information. New counselors must submit a new counselor application (Microsoft Word format / PDF format) instead. Existing counselors who want to teach additional merit badges must also submit a new counselor application.

The Balboa Oaks Merit Badge Counselor list is updated yearly, and will be provided at Round Table on Thursday, September 2, 2010. If you are a merit badge counselor, then please fill out and submit this form once a year to verify that we have your correct address, phone number, and list of merit badges you teach.
